3. Pre-Assessment Requirements and Attendance

3.1. To inform the assessment, parents, young people, and schools must complete and return all requested questionnaires and paperwork at least 48 hours prior to the scheduled appointment. If this information is not returned in time, an additional appointment may be required, incurring charges as per Clause 2.2.2.

3.2. Please arrive no earlier than 10 minutes prior to your appointment time. The clinic facilities do not have a dedicated reception area, and early arrival may disrupt ongoing clinical sessions. If you arrive late, the missed time cannot be made up at the end of the session.

4. Initial Consultation and Booking Channels

4.1. All appointment bookings, enquiries, and scheduling must be made exclusively via our official online booking portal at https://drkmfenton.co.uk or by emailing our administrative practice team at contact@drkmfenton.co.uk.

4.2. We offer a complimentary 15-minute telephone call for prospective new patient enquiries to gather preliminary information and determine whether an assessment with our clinic is clinically appropriate.

5. Payment Terms

5.1. Valid payment card details must be provided via our secure online booking system at the time of booking to reserve your slot. No charges will be made to your card upon initial reservation.

5.2. An invoice will be issued prior to your scheduled assessment. Full payment must be received at least 48 hours prior to the scheduled appointment time.

5.3. If payment is not settled 48 hours in advance, we reserve the right to charge the stored payment card on file for the full balance or cancel the reservation.

5.4. Payment plans are available upon request prior to booking confirmation.

6. Cancellations and Rescheduling

6.1. We require a minimum of 48 hours’ notice to cancel or reschedule an appointment. Cancellations made within 48 hours of the appointment time will incur the full appointment fee. This rule applies to all circumstances, including unexpected work commitments, illness, and family emergencies.

6.2. Our clinic spaces are managed by an independent third-party provider that operates strictly Monday–Friday (excluding bank holidays) and requires 24 hours' working notice for cancellations.

  • 6.2.1. If you cancel with 48 hours' notice to us, but that window falls outside the third-party room provider's operational hours, preventing us from giving them 24 hours' working notice, you will remain liable for the room booking fee.
  • 6.2.2. To avoid liability for room booking fees for appointments on a Saturday, Sunday, or Bank Holiday, cancellations or rescheduling requests must be submitted at least 48 working hours prior to the last business day preceding the weekend or holiday.

6.3. Rescheduling or cancellation requests must be sent directly via email to contact@drkmfenton.co.uk.

7. Reports, Letters, and Clinical Communication

7.1. Comprehensive reports will be completed and sent within 4 weeks of the final appointment, provided all required screening forms and paperwork have been fully completed and returned.

7.2. Reports and clinic letters will be securely emailed directly to the email address provided during booking. Please notify us in writing prior to appointment completion if you prefer an alternative delivery method.

7.3. We do not provide acute clinical advice, crisis triage, or ongoing medical management via email. Any medical updates must be submitted via our official forms issued as appropriate, and a dedicated follow-up appointment must be booked if medical discussion is required.

7.4. All communications, whether administrative, scheduling, or clinical, must be routed exclusively through our centralised administrative channel (contact@drkmfenton.co.uk). Dr Fenton's own business email address is strictly reserved for internal administrative, clinical governance, and staff operational use. Unsolicited direct patient emails sent to this address will be redirected or ignored.

7.5. Patients, parents, and guardians must respect professional boundaries at all times. Under no circumstances should patients or their families:

  • Attempt to contact Dr Fenton via her private personal email address, personal telephone numbers, home address, or personal social media accounts.
  • Contact, approach, or solicit Dr Fenton, her NHS colleagues, NHS management, or hospital departments regarding private practice matters, private fee disputes, or private clinical cases.
  • Use external NHS or personal channels to exert pressure, issue ultimatums, or bypass clinic administration.

8. Prescriptions and Medication Management

8.1. Repeat prescriptions issued IN PLACE of a formal review appointment can be provided where deemed clinically appropriate by the prescribing clinician. This incurs an administrative fee of £20, payable prior to issuance.

8.2. Repeat prescription requests must be submitted at least 2 working days before Thursday. Prescriptions are issued weekly on Thursdays. Requests should be emailed to prescriptions@drkmfenton.co.uk.

8.3. Prescriptions are routinely issued via digital partner pharmacies (Pharmacierge or CloudRx). If you prefer physical paper prescriptions sent via registered post to your home or chosen local pharmacy, please notify us in advance. An administration and postage fee will be payable.

8.4. Prescriptions will not be processed during clinic annual leave. Closure dates are regularly published in our email signatures and on our website. Patients must review their medication supply and submit requests well ahead of scheduled closures.

9. Confidentiality, Data Protection, and Records

9.1. Consultations and clinical records are treated with the strictest medical confidentiality. In accordance with professional standards (GMC) and statutory obligations, if a clinician determines there is a serious, imminent risk of harm to the young person, yourself, or others, or where required under child safeguarding protocols, we have an ethical and legal duty to notify relevant statutory agencies (e.g., GP, social services, or emergency services). Where clinically appropriate and safe, we will endeavour to inform you before sharing this information.

9.2. In line with General Medical Council (GMC) good practice guidelines for child and adolescent psychiatry, clinical summaries and diagnostic reports are routinely shared with the young person's registered NHS General Practitioner (GP). If you explicitly do not consent to sharing records with your GP, you must inform us in writing prior to your assessment; please note that withholding consent to inform a GP may restrict our ability to prescribe medication or enter Shared Care Agreements.

9.3. We operate in full compliance with the Data Protection Act 2018 and the UK General Data Protection Regulation (UK GDPR). Dr K M Fenton acts as the Data Controller. Full details regarding our legal bases for processing, retention periods, and your statutory rights (including access, rectification, and erasure) are set out in our Privacy Policy.

9.4. All electronic medical records, correspondence, and questionnaires are held securely on our cloud-based Electronic Health Record (EHR) platform, Semble, which is ISO 27001 certified, hosted in UK/EU data centres, and secured via multi-factor authentication (MFA).

9.5. To ensure our clinicians remain fully focused on the young person and family rather than manual notetaking, our practice utilises secure ambient clinical voice technology (such as an AI medical scribe) during appointments.

  • The software securely captures the audio dialogue of the consultation in real time to generate structured, accurate clinical notes and draft diagnostic reports for the clinician's review.
  • The technology complies fully with UK GDPR and medical confidentiality regulations. Audio recordings are encrypted in transit and at rest, processed strictly within secure UK/EU environments, and are permanently erased once the transcript has been verified. Audio recordings are never retained long-term or used to train third-party public AI models.
  • All generated documentation is reviewed, validated, and edited directly by Dr Fenton before being entered into the official medical record.
  • The use of ambient documentation is optional. If you do not wish for ambient voice technology to be used during your consultation, please inform us in writing prior to your appointment or inform Dr Fenton at the start of your session. Opting out will not prejudice your care or change the clinical time allocated to your appointment.

10. Feedback and Complaints

10.1. If you have concerns regarding administrative, room, or clinical services, we encourage you to discuss the matter directly with Dr Fenton or practice administration in the first instance, as many issues can be resolved promptly and informally.

10.2. If your concern cannot be resolved informally, please submit a formal complaint in writing to our Business Manager at contact@drkmfenton.co.uk, who will review and address your concerns.

Please clearly outline the nature of your concern, relevant dates, and your desired outcome.

10.3.We will acknowledge receipt of formal complaints in writing within 3 to 5 working days.

10.4.We will conduct a thorough investigation and provide a comprehensive written response within 20 working days. If an investigation requires additional time due to complexity or clinical record retrieval, we will keep you updated on progress.

10.5. External Escalation: If, following our formal written response, you remain dissatisfied with the internal outcome:

  • Professional Standards & Clinical Conduct: You may refer concerns regarding medical practice to the General Medical Council (GMC).
  • Data Protection: Concerns regarding health records and data privacy may be escalated to the Information Commissioner’s Office (ICO).
  • Independent Resolution: For commercial or private healthcare service disputes, complaints may be referred to an independent external mediation or adjudication provider.

11. Professional Conduct, Zero Tolerance, and Unacceptable Behaviour

11.1. Our clinic operates a strict Zero Tolerance policy regarding abusive, aggressive, harassing, threatening, or coercive behaviour directed towards our clinician, administrative staff, or associated third-party contractors.

11.2. Any attempt to intimidate the clinician or staff, including threats to publish unfounded allegations, attempts to interfere with Dr Fenton’s NHS employment or professional standing, extortion, harassment, or coercive demands, is strictly prohibited and represents an irretrievable breakdown of the doctor-patient relationship.

11.3. While we provide a transparent, formal internal complaints procedure (Clause 10), any false, malicious, or damaging statements published or communicated to third parties, regulatory bodies, NHS employers, or online platforms in bad faith will be treated as civil defamation.

11.4. In the event of a breach of Clauses 7.5, 11.1, 11.2, or 11.3, Dr K M Fenton reserves the right to:

  • Terminate the clinician-patient relationship immediately without refund and safely discharge the patient back to primary care (GP);
  • Report harassment, blackmail, stalking, or menacing communications directly to the Police and relevant statutory authorities under the Protection from Harassment Act 1997 and Malicious Communications Act 1988;
  • Instruct legal counsel to pursue civil remedies, including claims for damages, injunctive relief, and legal costs arising from defamation, tortious interference, or harassment.

12. Safeguarding and Appointment Accompaniment

12.1. Dr Kristy Fenton is committed to safeguarding and promoting the welfare of children, adolescents, and young adults in strict accordance with statutory child protection legislation and General Medical Council (GMC) guidance.

12.2. Parents or legal guardians are routinely present during clinical appointments to ensure emotional reassurance, shared clinical understanding, and safety.

12.3. In certain clinical contexts, it may be beneficial for the assessment process to include a brief intermission where the clinician and young person speak one-to-one. This will only ever occur where:

  • (a) Both the parent/guardian and the young person give explicit, informed consent during the appointment;
  • (b) Both the parent and young person feel completely comfortable proceeding with a one-to-one discussion;
  • (c) The parent remains immediately outside or on the premises throughout; and
  • (d) Either the patient or parent may request to end the intermission and resume joint consultation at any time.

12.4. Given that parents/guardians are present as standard, independent clinical chaperones are not routinely required. However, an independent chaperone can be arranged upon advance written request for in-person consultations.

12.5. While clinical consultations remain confidential, if disclosures or presentations indicate that a child or young person is suffering or at serious risk of significant harm, we have a statutory duty to notify relevant statutory authorities (such as the local authority Multi-Agency Safeguarding Hub [MASH] or emergency services). Wherever clinically appropriate and safe, we will discuss this with parents/guardians prior to making a referral.
